Mittelschmerz and fertility

Mittelschmerz is often felt on one side of the lower abdomen around the middle of a cycle. It may be a brief ache, cramp, or sharper sensation. The side can vary because either ovary may release an egg. Some people have it regularly, while others never notice it.

Pain timing alone cannot show whether an egg was released or whether the fallopian tubes, sperm, or other fertility factors are healthy. Endometriosis and ovarian cysts can also cause pelvic pain, particularly when discomfort is persistent, severe, or unrelated to the usual cycle pattern.

Record the day pain starts, its side, duration, intensity, bleeding, discharge, and other symptoms. If you are trying to conceive, combine this pattern with cycle tracking or an ovulation test rather than relying on pain alone.

For mild, familiar discomfort, rest, warmth, and a suitable non-prescription pain reliever may help. Check that a medicine is appropriate if you could be pregnant or have another health condition.

Get urgent help for sudden severe pelvic pain, fainting, fever, repeated vomiting, heavy bleeding, or pain with a possible pregnancy. Arrange a clinical review when pain persists, returns outside mid-cycle, worsens, or affects sex and daily activities.
