Miscarriage: How Do I Know If I Passed Everything?

Passing clots or tissue may happen during a miscarriage, and bleeding may gradually become lighter, but these changes do not prove that the uterus is empty. Some people have little bleeding, while others continue to bleed after the pregnancy has passed. Arrange the follow-up recommended by your maternity or gynaecology team, even if you feel physically better.

Cramping usually reflects the uterus contracting, and bleeding can occur as the pregnancy tissue separates and leaves the uterus. The pattern may change over time, so symptom improvement is reassuring but is not a confirmation of completion. Ongoing bleeding, pelvic pain, or a positive pregnancy test can mean that follow-up is still needed; they can also have other explanations.

A transvaginal ultrasound can assess the uterus and help the clinician decide whether further management is needed. Depending on your symptoms, examination, and preferences, the care team may discuss waiting for the miscarriage to pass, medical management, or surgical management.

Use sanitary pads while bleeding so you can observe the amount and pattern; avoid putting anything into the vagina until your clinician says it is appropriate. Rest as needed, drink fluids, and use only pain relief that your clinician or pharmacist has said is suitable for you. If you pass tissue, you can ask whether it should be kept for assessment, but do not delay urgent care to collect it.

Contact the service that diagnosed or treated the miscarriage to confirm the follow-up plan. The team may arrange an ultrasound, pregnancy-hormone testing, or a review of your symptoms. Ask when to expect bleeding to settle and what to do if a pregnancy test remains positive.

Seek urgent medical help for heavy or rapidly increasing bleeding, severe or worsening abdominal pain, fainting, marked dizziness, weakness, fever, chills, or unpleasant-smelling vaginal discharge. These symptoms can signal significant blood loss or infection and should not be assessed only at home.

Get emergency help for sudden severe pain, especially if it is mainly on one side, or pain that reaches the shoulder with dizziness or fainting. Such symptoms need prompt assessment because not every early-pregnancy emergency is explained by a miscarriage.

Ask how completion will be checked, when the review should happen, and which symptoms should prompt earlier contact. You can also ask whether an ultrasound or another assessment is appropriate, when to repeat a pregnancy test, and what options are available if tissue remains.
