Miscarriage and no period: what to know

Bleeding after a miscarriage is not the same as a menstrual period. Ovulation can happen before the first period, so pregnancy is possible if you have had sex without contraception. Stress, physical recovery and hormonal changes can also shift the cycle. If bleeding, pain or pregnancy symptoms continue, tissue remaining in the uterus is one possibility that needs assessment rather than guessing from the calendar.

A clinician may ask when bleeding stopped, whether pain or fever occurred, and whether a home pregnancy test remains positive. A transvaginal ultrasound can help assess the uterus and confirm whether the pregnancy has ended completely.

Take a pregnancy test if pregnancy is possible, and arrange a review if your period does not return within the timeframe your clinician gave you or if the test stays positive. Use sanitary pads while bleeding so you can observe the flow, rest as needed, and avoid putting anything in the vagina until a clinician says it is suitable. Treatment may be observation, medicine or a procedure, depending on the scan, symptoms and your preferences.

Seek urgent medical help for heavy bleeding, severe or worsening abdominal pain, faintness, shoulder-tip pain, fever, chills or feeling suddenly very unwell. These symptoms can signal significant blood loss, infection or a pregnancy outside the uterus and should not wait for a period to appear.

Ask whether you need a pregnancy test, blood tests or an ultrasound; what result would confirm that the uterus is clear; when to expect ovulation and menstruation; and which contraception or follow-up plan fits your situation.
