Miscarriage after a strong heartbeat: what to know

Cardiac activity shows that the embryo was developing when the ultrasound was performed. It cannot predict every later pregnancy outcome. Bleeding, cramps, reduced pregnancy symptoms, or a later scan showing no cardiac activity may be associated with miscarriage, but symptoms alone cannot establish the diagnosis.

Your clinician may compare the earlier scan with a new transvaginal ultrasound and may use pregnancy hormone tests when appropriate. Dates can be uncertain, so the timing and findings of each scan matter when interpreting a change.

Call your pregnancy care team and share the earlier scan date, the new symptoms, pain location, bleeding pattern, and any medical conditions. Do not blame yourself: ordinary movement, work, or emotional upset is not a reliable explanation for an early loss. Follow instructions about medicines and follow-up imaging.

If miscarriage is confirmed, you can discuss expectant, medical, and surgical approaches. The choice depends on your health, symptoms, ultrasound findings, and what feels acceptable to you. Ask about emotional support as well as physical follow-up.

Attend emergency care for severe pain, one-sided pain, shoulder-tip pain, fainting, fever, or bleeding that rapidly becomes heavy. These symptoms need prompt evaluation for ectopic pregnancy, infection, or blood loss, regardless of the earlier heartbeat.

Ask what the new ultrasound shows compared with the earlier scan, whether further testing is needed, how long to expect bleeding, and which physical and emotional symptoms should trigger urgent review.
