What should you know about mild spinal cord compression?

Spinal cord compression occurs when a mass, damaged bone, swelling or another process presses on the spinal cord or the nerves around it. In oncology, cancer that has spread to the spine can weaken a vertebra or create a growth near the cord. The word mild describes the current degree of pressure or neurological change; it does not predict how the problem will progress.

Possible early features include new or worsening back or neck pain, pain travelling around the chest or down a limb, tingling, numbness, heaviness, or weakness. Walking may feel less steady. Symptoms can develop gradually, so a person may not recognise the change at first.

Contact your oncology team or an urgent medical service as soon as you notice new spinal pain with altered sensation, weakness, or difficulty walking. The clinician may examine strength, reflexes and sensation and arrange urgent imaging, often an MRI, to identify the level and cause of pressure.

Treatment depends on the findings and your overall care plan. Options can include medicines to reduce swelling, radiotherapy to control a tumour, or surgery when pressure, instability or tissue damage requires it. A condition such as Multiple Myeloma can involve bones and may be relevant to the assessment.

Go to an emergency department immediately for rapidly increasing weakness, inability to stand or walk, new loss of bladder or bowel control, difficulty passing urine, or numbness around the inner thighs or buttocks. Keep movement gentle while arranging help, and tell the service that spinal cord compression is a concern. Do not wait for a routine appointment if neurological symptoms are changing.
