Mild heartburn in early pregnancy: what to know

Short answer
Mild heartburn in early pregnancy is common, but persistent, severe or unusual chest and upper-abdominal symptoms should be assessed.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Helpful habit
- Stay upright after meals and choose smaller portions
- Medicine check
- Ask a maternity clinician or pharmacist before taking treatment
Why it can happen
Heartburn is a burning feeling behind the breastbone or in the upper abdomen, sometimes with a sour taste or fluid coming back into the throat. Pregnancy hormones can relax the valve between the stomach and food pipe and slow digestion, allowing reflux. It may also be worsened by large meals, lying down soon after eating or pressure on the abdomen.
Heartburn can overlap with acid reflux, gastritis or a hiatus hernia, but the sensation alone cannot distinguish them. Chest pain can have causes unrelated to digestion, so describe its location, timing and associated symptoms clearly.
Comfort measures
Try smaller meals, eat slowly and remain upright after eating. Identify personal triggers such as spicy or fatty foods, caffeine or acidic drinks, and avoid eating close to bedtime. Raise the head of the bed if night symptoms occur. Check every medicine, antacid or supplement with a pharmacist, midwife or doctor before using it in pregnancy; treatment choices depend on your stage of pregnancy and health history.
When to seek care
Seek urgent help for crushing or severe chest pain, breathlessness, fainting, sweating with chest discomfort, vomiting blood or black stools. Contact your maternity team or clinician promptly if pain is severe, swallowing is difficult, vomiting prevents fluids, symptoms are new and persistent, or upper-abdominal pain occurs with severe headache, visual change or marked swelling.
Questions for your doctor
Which lifestyle changes fit my symptoms? Is a pregnancy-compatible medicine appropriate? Could another condition explain the pain? Which chest, swallowing or vomiting symptoms should prompt urgent review?
