What the technique means

During the Milch approach, a trained professional positions the arm and gradually guides it through a carefully chosen range. Muscle guarding can make the joint harder to move, so forceful attempts or home manoeuvres can worsen injury to bone, nerves, blood vessels, or nearby soft tissue.

Before and after the joint is reduced, the clinician checks circulation, sensation, movement, and possible fracture. Imaging may be needed, particularly after a fall or direct blow. Reduction is only the first part of care; the shoulder may need support, pain relief, rehabilitation, and a plan to reduce recurrence.
