What should you know about mid-cycle bleeding and pain?

Ovulation pain is linked with the release of an egg and irritation near the ovary. It is a presentation of ovulation for some people, but mid-cycle symptoms do not confirm that ovulation is the cause. Endometriosis can present with pelvic pain and bleeding changes, while an ovarian cyst can present with pain, pressure or an altered bleeding pattern.

A clinician may ask about cycle dates, pregnancy possibility, bowel or bladder symptoms, pain during sex and previous pelvic problems. Examination, pregnancy testing or imaging may be considered when the pattern is unclear or symptoms persist.

Keep a cycle diary with the first day of bleeding, spotting, pain location, severity, duration and triggers. Rest, use a heat pack and take only pain relief that is safe for you and used according to its instructions. If pregnancy is possible, take a pregnancy test and ask a clinician which medicines are appropriate.

Arrange an appointment if episodes keep returning, interfere with daily activity, occur after sex, or come with heavy periods, pain during sex, bowel symptoms or difficulty becoming pregnant. Bring your cycle record and medication list.

Seek urgent assessment for sudden severe or one-sided pain, fainting, dizziness, shoulder-tip pain, fever, vomiting, heavy bleeding or a positive pregnancy test with pain or bleeding. These symptoms can signal a problem requiring prompt treatment, so do not rely on the timing of the cycle to judge safety.
