Menstruation after hysterectomy

A hysterectomy removes the womb, so there is no womb lining to shed as a period. In a partial or subtotal hysterectomy, a small amount of the cervix may remain and light cyclical spotting can occasionally occur; the operation notes can clarify which type was performed. The ovaries may be left in place, meaning hormonal cycles and menopause symptoms can continue even though periods do not.

Light vaginal bleeding or discharge can be part of healing after surgery. New bleeding later on is not automatically a period. Conditions that led to surgery, such as fibroids, adenomyosis, endometriosis or womb-lining problems, may still be relevant to follow-up.

Follow the discharge instructions about activity, sex, bathing and vaginal products. Track the colour, amount and duration of any bleeding. Use pads during recovery if advised, rather than inserting products without clearance. Keep your operation details available and ask whether the cervix and ovaries were removed. Persistent pelvic pain or heavy bleeding deserves a clinical review.

Contact your surgical team promptly for bleeding that becomes heavier, bright-red bleeding with clots, worsening pelvic pain, fever, unpleasant-smelling discharge, wound changes or feeling unwell. Seek emergency help for severe pain, faintness, heavy bleeding, chest pain or difficulty breathing. Bleeding months or years after surgery should be assessed rather than assumed to be menstruation.
