Medical management of UTI

A clinician asks about burning or painful urination, urgency, frequent trips to the toilet, lower abdominal discomfort and changes in urine. Fever, vomiting or pain in the side or back can suggest that infection is affecting the kidneys rather than only the bladder. A urine sample may help confirm infection and guide antibiotic choice, particularly when symptoms are severe, recurrent, unusual, or do not improve. Treatment decisions also take account of pregnancy, kidney problems, allergies and medicines you already use.

Take prescribed medicine exactly as directed and complete the course unless the prescriber changes it. Drink fluids regularly unless you have been told to restrict them, and rest while symptoms settle. Do not use leftover antibiotics or share them, because the wrong medicine may delay recovery. A pharmacist or clinician can advise on suitable pain relief and whether your symptoms need testing. Keep track of worsening discomfort, fever, vomiting or reduced urine so you can describe the change clearly.

Seek prompt medical advice for fever, chills, vomiting, pain in the back or side, visible blood in urine, or symptoms that are worsening or not settling after treatment. Urgent assessment is particularly important during pregnancy, in children, in men, or for someone with significant kidney disease or a weakened immune system. Emergency help is needed if you become confused, faint, very drowsy, or unable to keep fluids down.

Ask whether a urine test is needed, which symptoms would suggest kidney involvement, when improvement should begin, and what to do if symptoms return. Confirm whether any regular medicine, allergy or pregnancy could change the treatment choice.
