Can measles occur without a rash?

A rash may not be present at the start of an illness, may be difficult to see on some skin tones, or may not develop in every presentation. Fever, cough, runny nose, sore eyes, tiredness, and swollen glands can have many causes, so the absence of a rash does not confirm or exclude measles.

A clinician considers the complete picture: timing, known exposure, vaccination history, travel, age, and examination findings. Do not use the missing rash alone to decide that contact with others is safe.

Call a GP or health service before attending if measles is possible. Explain the fever, respiratory symptoms, exposure history, and whether a rash has appeared. Follow instructions about where and when to be assessed, and keep away from crowded settings while waiting for advice.

Rest, drink fluids, and monitor the person’s alertness, breathing, and ability to drink. Record new symptoms and check immunisation information. Ask whether MMR vaccination advice is needed for the person or close contacts; recommendations depend on individual circumstances.

Seek urgent care for breathing difficulty, confusion, a seizure, severe drowsiness, inability to drink, or dehydration. Mention possible measles before arriving. Arrange prompt assessment for a baby, pregnancy, or weakened immunity, or if fever is worsening even without a rash.

Could this be measles before a rash develops, or another infection? How should we avoid exposing people during assessment? What symptoms should we monitor overnight? Is MMR vaccination appropriate? When should the person return to school, work, or childcare?
