What should you know about a high MCV blood test?

A high MCV means red blood cells are larger than average, and the result needs context from the rest of the blood count. It is a finding, not a diagnosis, and may be temporary, medication-related, or linked with a nutritional, digestive or medical problem.

Large red cells can appear with low vitamin B12 or folate, regular alcohol use, liver or thyroid problems, certain medicines, or changes in bone-marrow production. A high MCV can occur with anaemia, but it can also appear when haemoglobin is not low. Recent illness may alter some blood results, so timing matters.

Your clinician will consider haemoglobin, red-cell appearance, white cells, platelets, symptoms and previous results. Digestive conditions such as coeliac disease or Crohn’s disease can affect nutrient absorption and may be relevant in some people.

Book a review to discuss the complete report rather than reacting to the MCV number alone. Bring a list of medicines, supplements and alcohol intake, and mention numbness, balance changes, sore tongue, digestive symptoms, weight change or unusual fatigue. Your clinician may repeat the blood count or request vitamin, thyroid, liver and other checks. Avoid self-treating with high-dose supplements before the cause is assessed.

Seek urgent help for severe breathlessness, chest pain, fainting, new confusion, marked weakness, or heavy bleeding. Arrange prompt medical review for rapidly worsening fatigue, new neurological symptoms such as difficulty walking or persistent numbness, or a report showing several abnormal blood-cell types. A stable isolated result can usually be discussed at a planned appointment.
