Mastitis without redness: what to know

Redness is common in some cases but is not required. Inflammation may be deeper in the breast or less visible on darker skin, and early symptoms can be subtle. You may instead notice a warm patch, swelling, heaviness, a firm area, altered nipple appearance, discharge, or a fever. A breast lump is not automatically mastitis: cysts, blocked ducts, infection, and other conditions can produce overlapping signs. Examination is the safest way to sort these possibilities.

Arrange an assessment for a new or persistent change, even when the skin looks normal. Make a note of the area, its size or texture, the time it appeared, and whether symptoms vary with your cycle or feeding. Avoid squeezing a lump or using strong pressure. Seek urgent medical attention for fever with worsening illness, quickly increasing swelling, pus, severe pain, red streaking, blistering, or darkening skin. Prompt review is also needed for a persistent lump, bloody discharge, nipple inversion, dimpling, or an ulcer. Normal-looking skin does not make these findings safe to ignore.
