What should you know about mastectomy with expanders?

A tissue expander is an empty or partly filled implant with a valve. After mastectomy, a surgeon places it in the chest and adds sterile fluid during follow-up visits. The gradual expansion creates room for a permanent implant or may help prepare the area for another reconstruction method. Tightness, pressure and altered breast sensation can occur after fills. The expander is not usually the final shape, and the two sides may look different during the process. The timing and choice of reconstruction can be affected by chemotherapy before surgery, radiotherapy, skin condition and the mastectomy plan.

Before surgery, ask how often you will attend expansion visits and whether the device will be placed under or over muscle. Arrange comfortable clothing and help with tasks that involve pushing, pulling or lifting. Keep every wound and expansion appointment, follow activity limits and take prescribed pain relief as directed. Tell the team about increasing tightness, skin thinning, a visible valve or a change in the expander position. Contact the surgical team promptly for fever, spreading redness, drainage, wound separation, rapidly increasing swelling or pain that is not settling. A breast that becomes suddenly very tight, pale, dark or cold needs urgent assessment. Seek emergency help for severe breathlessness, chest pain, fainting or sudden facial swelling.
