Malleolar bursitis: what to know

How the structures involved in Bursitis differ from normal

Short answer

Malleolar bursitis is irritation of a fluid-filled sac beside an ankle bone, often causing local swelling, tenderness, and pain from pressure.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Typical site
Beside one of the ankle malleoli
Common aggravator
Repeated rubbing or pressure from footwear
Avoid
Puncturing, squeezing, or pressing directly on the swelling

What is happening around the ankle

A bursa cushions movement between soft tissue and bone. Around the malleoli, repeated rubbing from footwear, kneeling, contact, or an ankle injury can irritate the sac. The area may look puffy and feel sore when touched or when a shoe presses against it. Movement may be possible, although side-lying, walking in tight footwear, or ankle motion can be uncomfortable.

Joint Swelling around the ankle can have other causes, including injury or infection, so appearance alone cannot confirm bursitis. A clinician may examine the skin, ankle stability, range of motion, and tenderness, then decide whether further tests are needed.

Steps that may reduce irritation

Remove pressure from the tender area with roomier footwear, protective padding, and a temporary reduction in activities that rub the ankle. Elevate the leg when resting and use a wrapped cold pack for short periods if it feels soothing. Do not puncture or squeeze the swelling yourself.

A clinician may discuss Anti-Inflammatory Medication when suitable for your health history. If fluid persists or the diagnosis is uncertain, Joint Aspiration can help remove fluid for examination or relief. A Cortisone Injection may be considered in selected cases after infection has been excluded.

When to seek care

Get urgent assessment if the ankle becomes hot, very red, rapidly more swollen, or associated with fever or feeling unwell. Seek prompt care after a significant injury, if you cannot bear weight, or if the foot becomes numb, pale, or cold. Persistent drainage or an open sore over the swelling also needs review.

Questions for an appointment

Ask whether the swelling is in a bursa or the joint, whether infection or a fracture needs exclusion, how to protect the area in footwear, and which treatment is appropriate if it does not settle.

Questions people ask

Can shoes cause malleolar bursitis?

Yes. Repeated rubbing or pressure from a tight or rigid shoe can irritate the bursa beside the ankle bone.

Does ankle swelling always mean bursitis?

No. Swelling may arise from a sprain, joint inflammation, infection, or another problem, so persistent or hot swelling should be examined.
