What should you know about malignant cells in pleural fluid?

Short answer
Malignant cells in pleural fluid mean that cancer cells were identified in fluid collected from the space around a lung, so the next steps are to confirm the result, identify the cancer source and relieve symptoms.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Sample tested
- Fluid removed from the pleural space
- Main goals
- Identify the source, assess the lung and ease symptoms
What the result means
The pleural space is the thin space between the lung and chest wall. Cancer in this fluid can cause a malignant pleural effusion, which may press on the lung and make breathing uncomfortable. The cells may have come from a cancer that began in the lung, breast or the lining of the chest, among other possible sources; the fluid result alone does not always identify the origin.
A sample is usually obtained during thoracentesis, a procedure that removes fluid with a needle. A laboratory examines the cells and may use additional tests to help classify them. Imaging and, sometimes, a tissue biopsy help the team understand where the cancer began and how far it has spread.
What to do next
Arrange a prompt appointment with the oncology or respiratory team that ordered the test. Bring the report and a list of medicines, symptoms and previous cancer records. Ask whether more imaging, a biopsy or a repeat fluid sample is needed, and whether the fluid is affecting lung expansion.
Treatment is tailored to the cancer source and your breathing. Options may include draining the fluid, a procedure to reduce repeated build-up, cancer-directed treatment or symptom-focused support. Palliative care can be added alongside cancer treatment to help with breathlessness, discomfort, sleep and emotional strain.
When to seek urgent care
Seek emergency help for severe or rapidly worsening breathlessness, blue or grey lips, new confusion, fainting, or chest pain that is intense or persistent. Contact your treating team promptly for increasing breathlessness, fever, worsening cough or difficulty lying flat, because these symptoms can signal fluid returning, infection or another complication.
Questions for your doctor
Ask: What type of cells were found? Could the cancer have started outside the lung? Do I need imaging or a tissue biopsy? How much fluid is present, and what can ease my breathing? What symptoms should prompt an urgent call?
Questions people ask
Does this result identify the original cancer?
Not always. Cell features, imaging and sometimes a tissue sample are used together to identify the likely source.
Can malignant pleural fluid return?
It can return in some people. The team can discuss drainage or other measures if it causes breathing symptoms.
