Can malaria occur without fever?

A temperature reading is only a snapshot. Fever can come and go, be reduced by medicine, or be missed if a thermometer is inaccurate or symptoms are just beginning. Some people with malaria have an unusual presentation, especially after taking preventive medicine or when immune responses differ. Chills, sweating, headache, tiredness and muscle aches can still occur. No single symptom confirms or excludes malaria, so recent travel to a risk area is a key part of the assessment.

Contact a clinician promptly if you have malaria-like symptoms after travel, even if your temperature is normal today. Give the full itinerary, return date, mosquito exposure, medicines and the timing of symptoms. A blood test may be needed, and clinicians can advise whether testing should be repeated if the first result does not explain the illness. Avoid taking leftover antibiotics or antimalarial medicine without medical guidance.

Go to emergency care for confusion, fainting, seizures, severe drowsiness, breathing difficulty, persistent vomiting or marked weakness. A normal temperature does not make these warning signs safe to watch at home. Seek same-day advice for chills or a flu-like illness after travel to a malaria-risk area, particularly during pregnancy or for a child.

Ask whether your symptoms and travel history warrant a blood test, whether medicines could have changed your temperature, and which changes require emergency care. Discuss mosquito protection and malaria prophylaxis before future travel.
