What should you know about lytic bone metastases?

The word lytic describes bone loss caused by increased activity of cells that remove bone. Affected areas may occur in the spine, pelvis, ribs or long bones. Pain can be dull, persistent or worse with movement, but some lesions cause no symptoms and are found during imaging. A weakened bone can fracture after a minor injury. Cancer types that may spread to bone include breast, lung and prostate cancer, among others; the original cancer and the pattern on imaging guide the next steps.

Your oncology team may combine your symptoms, examination, blood tests and imaging such as X-ray, CT, MRI or a bone scan. Treatment is chosen around the cancer type, the bone involved and your overall health. Options may include medicine that slows bone breakdown, cancer-directed treatment, radiation to a painful or high-risk area, and surgery or a brace when stability is threatened. Pain treatment can include several approaches. Do not start supplements or strenuous exercise without checking first, because the plan may need to account for kidney function, fracture risk or other medicines.

Contact your cancer team promptly for new or worsening bone pain, pain that wakes you, difficulty walking, or a limb that looks swollen or changed. Seek emergency help for sudden severe back or neck pain, new weakness or numbness, loss of bladder or bowel control, or inability to stand after an injury. These symptoms can signal spinal cord pressure or a fracture and need rapid assessment.
