Lupus and shortness of breath: what should you know?

Lupus may affect the lining around the lungs, the lung tissue, blood cells, heart, or muscles used for breathing. Breathlessness can also result from infection, asthma, acute bronchitis, chronic obstructive pulmonary disease, bronchiectasis, angina, atrial fibrillation, or another condition. Notice whether symptoms occur at rest or with activity, begin suddenly or gradually, and come with cough, fever, wheezing, chest pain, palpitations, ankle swelling, or unusual tiredness.

Arrange prompt medical assessment for new or persistent breathlessness, especially if it limits walking, speaking, or sleep. Write down the onset, triggers, associated symptoms, recent infections, travel, and every medicine you take. Sit upright, rest, and avoid exertion while waiting for advice. Do not change lupus medicines or use someone else’s inhaler. A clinician may check oxygen levels, heart and lungs, blood tests, and other investigations based on your symptoms.

Call emergency services for severe breathing difficulty, blue or grey lips, inability to speak in full sentences, fainting, sudden chest pain, confusion, or rapidly worsening symptoms. Treat facial or throat swelling, hives, and breathing difficulty as an emergency because they may indicate anaphylaxis.

Ask whether the pattern suggests lupus involvement, anaemia, infection, asthma, or a heart cause; which tests are needed; and what action plan to follow if breathing changes. Ask whether any current medicine could contribute to the symptom.
