What should you know about lung hypertension?

Lung hypertension is raised pressure in the blood vessels that carry blood between the heart and lungs, and it needs medical assessment because it can strain the heart. It is also called pulmonary hypertension. This is different from hypertension, which refers to pressure in the arteries throughout the body. The two conditions can occur together, but one does not automatically cause the other.

Narrowed, stiff or blocked blood vessels in the lungs make it harder for the right side of the heart to push blood forward. Breathlessness during activity, tiredness, dizziness, chest discomfort, a fast heartbeat or ankle swelling can occur. Some people have few symptoms early on, so a clinician may investigate an unexpected finding or a condition that can affect the heart and lungs. Possible contributors include heart disease, lung disease, low oxygen during sleep, blood clots and some medicines or inherited conditions. The cause matters because treatment depends on the type of pulmonary hypertension.

Arrange an appointment if breathlessness, reduced exercise tolerance or swelling is new, persistent or worsening. Bring a list of medicines and supplements, details of previous heart or lung problems, and a record of symptoms such as when they start and what activity triggers them. Assessment may include oxygen measurement, blood tests, an electrocardiogram, chest imaging and an echocardiogram; specialist testing may be needed to confirm the diagnosis and identify its cause. Take prescribed treatment as directed, avoid stopping heart or blood-pressure medicines without advice, and ask whether activity, travel or altitude needs adjusting.

Seek emergency help for severe or rapidly worsening breathlessness, fainting, chest pain that does not settle, blue or grey lips, confusion, or coughing up blood. Sudden breathlessness with sharp chest pain may signal a blood clot and needs prompt assessment. New one-sided weakness, facial drooping, trouble speaking or sudden loss of balance can indicate a stroke; call emergency services immediately. Do not drive yourself if you feel faint, severely short of breath or confused.

Ask what type of lung hypertension is suspected and what may be causing it. You can also ask which tests are needed, whether a heart or lung specialist should review you, which symptoms should trigger urgent help, and how to take medicines safely. If sleep-related breathing problems, previous clots or a heart condition may be relevant, mention them explicitly.
