When lung cancer spreads to the bones

Bone metastases are areas where lung cancer cells have established beyond the original tumour. They may occur in the spine, ribs, pelvis, or other bones. Pain is possible, but some bone changes are found on imaging before symptoms begin. A new ache can also result from arthritis, strain, or another condition, so pain alone cannot identify its cause.

Doctors may use scans and blood tests to assess the bones and check for effects on calcium or bone stability. The team considers the number and position of affected areas, fracture risk, nerve pressure, the lung cancer type, and your general health when planning care.

Tell the cancer team about new, persistent, or worsening bone pain, pain that wakes you, reduced movement, or a change in walking. Ask whether you should adjust lifting, exercise, or other activities until the bones have been assessed. Do not wait for a routine visit if symptoms are progressing quickly.

Care may include medicines for the cancer, pain relief, bone-strengthening treatment, radiotherapy to a painful area, or surgery to support a weakened bone. Physiotherapy and Palliative Care can help preserve function and ease symptoms alongside cancer treatment.

Seek emergency care for sudden severe back or neck pain, new leg weakness or numbness, loss of bladder or bowel control, inability to bear weight, or a suspected fracture. These can signal spinal cord pressure or structural injury, where rapid assessment matters.

Ask which scan is needed, whether a bone is at risk of breaking, and which activities remain safe. Discuss pain relief, radiotherapy, bone-strengthening medicine, warning signs of nerve pressure, and how treatment response will be followed.
