Lung cancer can cause shortness of breath when a tumour, fluid, inflammation, or reduced lung function interferes with breathing, but breathlessness has many other possible causes. The sensation may appear during activity, at rest, or when lying flat. Its pattern, speed of change, and accompanying symptoms help a clinician decide what needs checking. A breathless feeling alone cannot establish whether someone has lung cancer.

What it can mean

Breathing may become harder if an airway is narrowed, part of the lung is affected, or fluid collects around the lung. Lung cancer can also be associated with a persistent cough, chest discomfort, coughing blood, wheezing, tiredness, or unexplained weight loss. These features can also occur with conditions such as asthma, COPD, bronchiectasis, acute bronchitis, anaemia, or heart problems. A clinician may listen to your chest and review your history before deciding whether tests such as chest imaging or breathing tests are appropriate.

What to do next

Arrange a medical assessment if breathlessness is new, persistent, gradually worsening, or limiting ordinary activities. Note when it began, what brings it on, whether it occurs at night or while lying down, and whether you have cough, fever, chest pain, noisy breathing, or leg swelling. Bring a list of medicines and mention smoking or vaping history, previous lung conditions, recent infections, and any known cancer diagnosis. Do not start or stop inhalers, oxygen, or other treatment without medical guidance.

When to seek urgent care

Seek emergency help now for severe or rapidly worsening breathlessness, blue or grey lips, fainting, confusion, inability to speak in full sentences, or severe chest pain. Coughing up more than a small streak of blood, or breathlessness with sudden one-sided leg swelling, also needs urgent assessment. Sit upright, loosen tight clothing, and use a prescribed reliever medicine according to your action plan while help is arranged. Do not drive yourself if you are severely breathless or faint.
