What should you know about a lump at the top of the neck near the base of the skull?

Upper-neck lumps can be related to lymph nodes responding to irritation or infection of the scalp, ear, throat, or nearby skin. A blocked skin gland, cyst, or muscle-related swelling can feel similar from the outside. Notice whether it is tender, mobile, soft, firm, warm, or attached to the deeper tissues. A lump can be a presentation of lymphoma or head and neck cancer, but its position cannot diagnose either condition. Thyroid problems are usually assessed lower and toward the front of the neck, though the full neck examination matters.

Use a mirror or ask someone to help you identify the exact spot, then note the date, tenderness, and any visible skin change. Check for scalp irritation, ear symptoms, sore throat, dental symptoms, or recent infection. Avoid repeated pressing, tight clothing over the area, and home attempts to open it. Book an examination when the lump remains, grows, becomes firm or fixed, or appears without an understandable trigger.

Seek urgent care if swelling interferes with breathing or swallowing, spreads quickly, follows a serious injury, or occurs with marked weakness. Prompt medical review is needed for persistent fever, drenching night sweats, unexplained weight loss, a non-healing scalp or skin sore, facial weakness, or new hearing or voice changes.

Ask which structures lie under the lump, whether the scalp, ear, throat, and teeth should be examined, and whether imaging is useful. Ask how to describe changes in size or firmness and what timeframe should prompt a follow-up appointment.
