What should you know about a lump on a newborn neck?

A neck lump in a newborn can come from normal development, a fluid-filled structure, an inflamed area, or a lymph node reacting to irritation or infection. The exact location matters: a lump near the front of the neck is assessed differently from one near the hairline or shoulder. A clinician may ask when it appeared, whether it is growing, and whether the skin is red or warm. Cancer is an uncommon explanation for a newborn lump, and the lump itself does not establish a diagnosis.

Assessment may include watching the baby feed, feeling the lump, and arranging imaging when the examination leaves questions. Conditions such as lymphoma, thyroid cancer, head and neck cancer, or cancer of unknown primary are specialist diagnoses, not conclusions that can be made from appearance alone.

Arrange prompt medical assessment and take a clear note of the lump’s size, position, colour and movement. Do not squeeze, massage, puncture or apply creams unless a clinician advises it. Keep normal feeding routines if the baby is comfortable, and bring details about birth, recent illness, fever, skin changes and wet nappies. If a thyroid-related cause is found, the care team can explain whether treatment or observation is needed; surgery such as thyroid removal is considered only for selected diagnoses.

Seek emergency help if the baby has noisy or difficult breathing, blue or grey lips, choking, marked sleepiness, repeated vomiting, inability to feed, or rapidly increasing swelling. Contact a clinician urgently for fever in a newborn, redness spreading over the skin, pus, or a lump that becomes tense and tender. These signs need prompt assessment because they may affect breathing, hydration or infection control.

Ask what structures lie beneath the lump, whether an ultrasound is appropriate, what changes should be watched at home, and when the baby should be reviewed. You can also ask which symptoms would require emergency care and whether a paediatric or head-and-neck specialist should be involved.
