Lump on the Neck That Will Not Go Away

Some neck swellings follow an infection and gradually settle, while others are cysts, thyroid nodules, salivary or skin problems, muscle-related changes, or deeper masses. A lump that persists cannot be classified reliably by its tenderness or by how it feels. A persistent neck lump can be a presentation of head and neck cancer, lymphoma, thyroid cancer, or cancer of unknown primary, but persistence does not prove that any of these is the cause.

The clinician will ask about its onset and change, examine the mouth and throat, and feel the thyroid, lymph-node areas and surrounding tissues. Further tests may be recommended when the examination does not explain it.

Arrange a medical appointment rather than continuing to monitor an unexplained lump indefinitely. Note its location, approximate size, firmness, mobility and whether it changes with swallowing or head movement. Mention recent infections, dental problems, skin irritation, injury, smoking or alcohol exposure, and any medicines, because these details help shape the assessment.

Seek urgent help for breathing difficulty, inability to swallow saliva or fluids, rapidly worsening swelling, or a sudden voice change with breathing problems. Request prompt review for a lump that is growing or fixed, or for accompanying weight loss, night sweats, persistent hoarseness, one-sided ear pain, coughing blood or difficulty swallowing. These symptoms set the urgency of assessment; they do not tell you the diagnosis on their own.

Ask what could explain the persistence, which structure appears involved, and whether imaging or a needle sample is needed. Ask what the examination has ruled out, how results will be communicated, and what symptoms mean you should seek help sooner. If the thyroid is involved, ask what treatment choices are available besides or before surgery.
