Lump on a neck muscle or tendon: what to know

A prominent neck muscle can feel like a cord when it turns the head, and tension or a recent strain may make nearby tissue sore or tight. A tendon attachment, a small fluid-filled structure, or a reactive lymph node can also feel like a lump. A lump that changes when you move or tense the neck may be connected with muscle, but movement does not prove its cause. Head and neck cancer and lymphoma can present with a neck lump; thyroid cancer is another condition clinicians may consider when the swelling is lower and toward the front. Cancer of unknown primary is considered when a neck mass is found without a clear origin.

Describe pain, movement, recent exercise or injury, infection, and whether the lump is in the muscle itself or just beneath the skin. Avoid forceful massage until it has been assessed.

Book a clinical review if the lump lasts, becomes more noticeable, feels fixed, or cannot be linked to a short-lived strain. The clinician may compare both sides, assess neck movement, examine the throat and lymph nodes, and request imaging or a needle sample when appropriate. Resting an irritated muscle and avoiding repeated probing may reduce discomfort, but these measures should not delay review of a persistent mass.

Seek urgent help for breathing difficulty, inability to swallow, rapidly expanding swelling, severe neck pain after an injury, or fever with spreading redness. Arrange prompt review for a hard or immobile lump, progressive enlargement, persistent hoarseness, unexplained weight change, drenching night sweats, or coughing blood.

Ask whether movement or muscle tension changes the finding, whether the lump is within muscle or a separate structure, whether imaging is needed, and what symptoms would change the urgency of follow-up.
