Lump on neck and headache

A lump may be a reactive lymph node responding to an upper-airway infection such as a common cold, COVID-19 or allergic rhinitis. Sinus inflammation can produce facial pressure and headache, while dehydration and heat exhaustion can cause headache with thirst, weakness or dizziness. Generalised anxiety disorder and sustained neck-muscle tension may also contribute to head pain, although they do not explain every lump. Dengue fever can cause fever and headache, particularly with marked body aches, but a neck lump should still be examined rather than attributed to one diagnosis from symptoms alone.

Note when the lump appeared, whether it is painful or moving under the skin, and whether it changes in size. Drink fluids, rest in a cool place if heat exposure is relevant, and use a labelled pain reliever that is safe for you. Do not press or repeatedly check the lump. Book an assessment if it persists, grows, or is accompanied by fever, night sweats, weight change, or ongoing headache. A clinician may examine the scalp, mouth, throat, nose and neck and decide whether further tests are needed.

Seek emergency care for a sudden severe headache, weakness, fainting, confusion, seizure, stiff neck with fever, repeated vomiting, difficulty breathing, or swelling that is rapidly enlarging. Prompt review is also needed for a severe headache with unusual bruising or bleeding, especially after travel or mosquito exposure.

Ask what structure is enlarged, whether the headache pattern suggests infection or muscle tension, how long observation is reasonable, and which symptoms should bring you back sooner.
