Lump on neck at the hairline: what to know

The hairline contains follicles and oil glands, so an ingrown hair, inflamed follicle, or skin cyst can create a rounded bump. Lymph nodes sit nearby and may become enlarged during a scalp, ear, throat, or skin infection. A tender lump with warmth or surface redness suggests local inflammation, while a firm lump under intact skin may need a closer examination. Head and neck cancer and lymphoma can present with neck lumps; thyroid cancer is usually assessed in the lower front of the neck, and cancer of unknown primary is considered when a mass is found without an obvious starting site.

Do not diagnose the lump from touch alone. Note whether it moves, whether the skin changes, and whether scalp irritation, ear symptoms, sore throat, or dental problems appeared around the same time.

Make a routine clinical appointment if the lump remains, enlarges, recurs, or has no clear skin-related explanation. A clinician can inspect the scalp and skin, examine the ears, mouth, throat, and neck, and decide whether imaging or sampling is useful. Keep the area clean, avoid picking at it, and take a dated photograph only if that helps you compare visible change.

Get urgent help if swelling affects breathing or swallowing, spreads quickly with severe redness, or is associated with feeling seriously unwell. Seek prompt medical review for a hard or tethered lump, unexplained bleeding or ulceration, persistent hoarseness, drenching night sweats, unexplained weight change, or a lump that continues to grow.

Ask whether the lump appears to arise from the skin, a follicle, muscle, or a lymph node; whether an infection is evident; what would justify imaging or a needle sample; and how soon it should be reviewed if it changes.
