What should you know about lump on neck behind ear?

A lump behind the ear may reflect a nearby lymph node or skin problem, while a persistent, hard, or growing swelling requires an examination. Ear, scalp, throat, dental, and skin problems can all affect tissues in this region.

The area behind the ear contains lymph nodes, skin, connective tissue, and the bony region of the skull. A node may enlarge while responding to an ear infection, scalp irritation, sore throat, or nearby skin inflammation. A cyst or irritated hair follicle can feel more superficial and may have a skin opening or tenderness. A deeper, firm, fixed, or enlarging mass needs assessment. Head and neck cancer and lymphoma can present with a neck mass; the location alone cannot distinguish those conditions from benign causes. A clinician may also consider the thyroid area or an unexplained primary source when the examination suggests it.

Assessment usually includes looking at the ears, scalp, mouth, and throat, feeling the lump and nearby nodes, and deciding whether imaging or a sample is useful.

Check the skin without repeatedly pressing the swelling. Note its position, approximate size, tenderness, movement, and any change over time. Look for recent ear pain, discharge, scalp sores, dental symptoms, or throat illness. Do not try to drain a lump yourself. Arrange a medical appointment if it persists, grows, feels hard or fixed, or has no obvious nearby trigger. Take note of medicines and recent infections so the clinician has the surrounding context.

Seek urgent care for trouble breathing, inability to swallow, rapidly spreading swelling, severe ear or neck pain with marked illness, or new weakness of the face. Seek prompt review for a growing or persistent lump, a hard or fixed mass, unexplained bleeding, persistent hoarseness, or significant swallowing difficulty. The region is close to the ear, throat, and important nerves, so new functional changes should not wait for routine monitoring.

Ask whether the lump feels like a lymph node, cyst, or another structure. Ask whether the ear, scalp, teeth, or throat could explain it. If it persists, ask whether ultrasound or another examination is appropriate, whether a sample is needed, and which symptoms should trigger earlier follow-up.
