What should you know about a lump on the back of the neck?

A lump behind the neck can have several causes, so its feel, duration, changes, and accompanying symptoms help guide assessment. It may be a reactive lymph node after an infection, a skin or soft-tissue problem, or a less common condition needing further investigation. A lump is not, by itself, a diagnosis of cancer.

The back of the neck contains lymph nodes that can enlarge while the immune system responds to an infection affecting the scalp, skin, throat, or nearby tissues. A tender lump that appears during an illness may behave differently from a firm, fixed, or steadily enlarging mass. Skin cysts and other non-cancerous growths can also occur in this area. Less commonly, a neck mass may be associated with lymphoma, head and neck cancer, thyroid cancer, or cancer whose starting site is not found; these conditions present with a lump, but a lump does not establish any of them.

Notice whether the area is painful, warm, red, mobile under the skin, or changing. Also record fever, night sweating, unexplained weight change, voice change, swallowing difficulty, or a sore that does not heal.

Do not squeeze, repeatedly press, or try to drain the lump. Note when you first noticed it, whether it followed a cold or skin irritation, and whether its size or position has changed. Arrange a clinical examination if it persists, grows, feels hard or fixed, or has no clear recent trigger. The clinician may examine the scalp, mouth, throat, neck, and thyroid, then decide whether observation, imaging, blood tests, or a sample is appropriate.

Seek prompt medical care for a lump with breathing or swallowing difficulty, rapidly increasing swelling, marked redness with fever, severe pain, or new weakness. Arrange a timely appointment for a persistent or enlarging lump, especially when it is firm, fixed, or accompanied by ongoing hoarseness, unexplained weight change, drenching night sweats, or a non-healing mouth or skin sore. Urgent symptoms should be judged by their severity, not by assuming the lump is cancer.

Ask what structures could be involved, what examination findings matter, and whether the lump appears to be a lymph node or a skin-based mass. You can ask how long observation is reasonable, what change should trigger review, and whether imaging or tissue sampling is needed. If thyroid disease is considered, ask whether thyroid testing or a thyroid scan is relevant.
