What should you know about a lump in the skin on the neck?

A surface lump may sit in the skin, under the skin, or over a deeper lymph node. A blocked pore, follicle inflammation, insect bite, cyst, or minor injury can cause redness, warmth, tenderness, or a visible opening. Lymph nodes can also be felt near the skin when they enlarge during an infection of the scalp, throat, ear, or teeth. A neck lump can be a presentation of lymphoma, head and neck cancer, thyroid cancer, or cancer of unknown primary, but a skin lump cannot be classified by appearance alone. A clinician may need to feel it and inspect the surrounding skin.

Do not squeeze, pierce, or repeatedly press the area; this can irritate skin and make assessment harder. Photograph visible changes and note redness, drainage, pain, growth, and whether the lump moves with the skin or separately beneath it. Book an examination when it persists, returns, becomes firmer, or has no obvious trigger. The clinician may check nearby lymph nodes and examine the mouth, throat, ears, and thyroid. Ultrasound or a needle sample may be considered according to the findings.

Seek urgent care if swelling spreads quickly, breathing or swallowing becomes difficult, or the area is very painful with fever. Arrange prompt review for a firm or fixed lump, unexplained bleeding or ulceration, persistent drainage, a lump above the collarbone, or associated voice change, night sweats, or unexplained weight loss. These signs call for assessment; they do not by themselves establish a cancer diagnosis.
