What should you know about a lump in the neck when turning your head?

Head movement can stretch the sternocleidomastoid muscle and alter the outline of lymph nodes, salivary tissue or the thyroid. A lump may follow an infection, inflammation, a cyst or another benign change. Head and neck cancer can present with a neck mass, while lymphoma can affect lymph nodes. Thyroid cancer may appear as a thyroid-area lump. A neck mass with no clear starting site can prompt a specialist work-up for Cancer of Unknown Primary. The position in which you notice it cannot determine which explanation applies.

Write down when you first noticed the finding and whether it is present with your head straight. Note tenderness, firmness, growth, recent infections and symptoms involving the throat, voice or swallowing. Bring a list of medicines and relevant tobacco exposure. A clinician may inspect the mouth and throat, feel the thyroid and lymph nodes, and choose imaging or needle sampling when indicated. Avoid forceful self-examination.

Call for urgent help if the swelling interferes with breathing or swallowing, appears rapidly, or is accompanied by severe illness. Seek prompt assessment for persistence, enlargement, hardening, ongoing hoarseness, swallowing pain, unexplained weight loss, fever or night sweats.

Ask which neck structure lies beneath the lump, whether head rotation changes its position or only its visibility, and whether you need an ultrasound or another test. Ask what result would require follow-up and how soon to recontact the clinic.
