What should you know about a lump in the neck when you turn your head?

Turning your head tightens muscles and shifts skin, glands and other neck structures, so a lump may stand out in one position. It might be an enlarged lymph node, cyst, thyroid nodule or another mass. Head and neck cancer may present as a neck mass, and lymphoma may involve lymph nodes. A lump near the thyroid can be associated with thyroid cancer. These conditions cannot be separated by whether a lump is easier to feel while looking to one side. An unexplained mass may also lead to assessment for Cancer of Unknown Primary.

Look once in a relaxed position and once while turning your head, without repeatedly pressing the area. Note the side, position, tenderness, change in size and duration. Tell the clinician about sore throat, dental infection, recent illness, voice change, swallowing symptoms, tobacco exposure or constitutional symptoms. Examination can cover the mouth, throat, thyroid and lymph nodes; imaging or a needle sample may be advised according to the findings.

Seek emergency help for breathing difficulty, inability to swallow, or fast swelling of the neck. Seek prompt medical review for a lump that persists or grows, or for new hoarseness, swallowing difficulty, persistent throat pain, fever, night sweats or unexplained weight loss.

Ask whether the lump follows a muscle, lymph-node chain or thyroid area; whether its movement with head turning is clinically meaningful; and which test can clarify its structure. Ask how to monitor it and when to return if it changes.
