Lump in neck that hurts to touch

Tenderness may occur when a lymph node reacts to an infection in the throat, teeth, ear, or skin. A cyst, irritated tissue, or a problem near the thyroid can also create a noticeable swelling. Pain alone cannot identify the cause. A neck lump can be a presentation of head and neck cancer, lymphoma, thyroid cancer, or cancer of unknown primary, but a lump does not establish any of these diagnoses.

Notice whether it is under the jaw, along the side of the neck, near the collarbone, or at the front of the neck. Record when it appeared, whether it changes in size, and whether swallowing, speaking, or turning your head affects it.

Arrange a clinical assessment if the lump does not settle, keeps enlarging, returns, or has no clear recent infection. A clinician may examine your mouth, throat, ears, thyroid area, and lymph-node regions. They may recommend imaging or a sample if the examination leaves questions. Do not repeatedly squeeze or massage the area, because this can increase soreness and make changes harder to judge.

Seek urgent help for trouble breathing, inability to swallow liquids, rapidly increasing swelling, or a muffled voice. Prompt medical review is also appropriate for unexplained weight loss, drenching night sweats, persistent fever, coughing blood, or a lump above or below the collarbone. These signs do not prove cancer; they indicate that examination should not be delayed.

Ask which structure seems enlarged, whether an infection could explain the tenderness, what examination or scan is appropriate, and how long you should monitor it. If testing is suggested, ask what the result could clarify and when you will receive the next step.
