What should you know about a lump at the bottom of the neck?

The bottom of the neck includes the lower cervical lymph nodes, the thyroid region, muscles, and tissues leading toward the collarbone and upper chest. A node can enlarge while the immune system responds to a throat, dental, skin, or chest infection. A thyroid swelling may rise during swallowing. A lump that is soft and mobile may have a different cause from one that is hard, deep, or fixed. Head and neck cancer, lymphoma, and thyroid cancer can present with neck swelling, but a bottom-of-neck lump is not a diagnosis and many causes are not cancer.

Book an assessment for a new lump that remains after an illness improves, becomes larger, feels firm, or is difficult to move. Before the visit, check whether it changes with swallowing, note tenderness and skin changes, and write down recent infections, dental problems, or injuries. The clinician may examine the neck, mouth, throat, and thyroid. Depending on those findings, an ultrasound, blood tests, or a needle sample may be considered. Avoid trying to drain or manipulate the lump yourself.

Get urgent help for breathing difficulty, inability to swallow saliva, rapidly increasing swelling, or severe pain with fever. Arrange timely medical review for a lump that is persistent, progressively enlarging, hard, or fixed, or for a swelling accompanied by unexplained weight loss, drenching night sweats, coughing blood, or ongoing hoarseness. These warning signs affect how quickly you should be assessed; they cannot determine the cause on their own.

Ask whether the lump seems to arise from a lymph node, thyroid, skin, or deeper tissue, and what examination or scan would clarify that. Ask how soon results should be reviewed, whether a recent infection changes the plan, and which symptoms should bring you back sooner.
