What the pain may mean

Ovulation pain, also called mittelschmerz, may feel like an ache, pressure or brief sharper sensation on one side of the lower back or pelvis. It can occur with mild lower abdominal discomfort or a small amount of spotting. The timing may help: cycle-related pain tends to follow a familiar pattern and improve without steadily worsening. Lower back pain can also come from muscles, the urinary tract, bowel, or a gynaecological condition. Endometriosis can cause pelvic or back pain that follows the menstrual cycle, while ovarian cysts may cause one-sided pressure or pain.

What you can do

Record when the pain starts, which side it affects, how long it lasts, and whether it appears with bleeding, discharge, urinary symptoms or bowel changes. Rest, gentle movement and a warm compress may ease a mild ache. Use an over-the-counter pain medicine only if it is suitable for you and follow the label. Avoid pushing through pain that becomes sharper during activity or sex. If the pattern is new, keeps returning, interferes with daily life, or pregnancy is possible, arrange a clinical assessment.

When to seek care

Seek urgent medical help for sudden severe pain, fainting, marked dizziness, heavy bleeding, fever, repeated vomiting, a rigid or very tender abdomen, or pain with a positive pregnancy test. Arrange prompt care if pain persists, worsens, occurs with unusual vaginal discharge, burning when urinating, or pain during sex. These features need an examination because the cause may not be ovulation.
