Understanding the timing

Ovulation-related discomfort may occur on one side of the lower abdomen when an ovary releases an egg. It can feel like a twinge, ache, or cramp and may recur at a similar point in the cycle. The timing alone cannot confirm ovulation. Ovarian cysts present with pelvic pain and can sometimes cause sudden or one-sided symptoms. Endometriosis, adenomyosis, and fibroids may produce pain that follows a cycle but lasts longer or affects other parts of the month. Pelvic inflammatory disease presents with pelvic pain and may include fever or unusual discharge. Ectopic pregnancy presents with pelvic pain when pregnancy is possible, even if the timing seems close to a period.

Compare the pain with your cycle dates, but also note duration, side, bleeding, discharge, fever, nausea, bowel symptoms, and urinary symptoms. A change from your usual pattern is useful information for a clinician.

How to manage it

Rest, use gentle heat if it feels soothing, and keep hydrated. Track the symptom across cycles, including which side hurts and whether it limits normal activity. If pain is mild and settles, discuss suitable pain relief with a pharmacist or clinician, especially if you may be pregnant or have other health conditions. Arrange an appointment when episodes are becoming stronger, lasting longer, or occurring outside the expected cycle window.

When it is urgent

Seek urgent care for severe or rapidly worsening pain, fainting, marked dizziness, shoulder-tip pain, heavy bleeding, fever, or repeated vomiting. If pregnancy is possible, one-sided pain or bleeding needs prompt assessment because ectopic pregnancy can be serious. Arrange routine review for recurring pain that interferes with work, sleep, sex, or exercise.
