Lower abdominal pain during sex: what to know

Lower abdominal pain during sex can come from pelvic muscle tension, dryness, infection, or a condition affecting the uterus or ovaries. Pain may occur with deep penetration, at the start of sex, or afterward. Its location, timing, severity, and accompanying symptoms help a clinician work out what needs checking.

Pain with deep penetration can be linked to irritation or inflammation in the pelvis. Pelvic inflammatory disease may cause lower abdominal pain, pain during sex, unusual discharge, or fever. Endometriosis and adenomyosis can cause pelvic pain that follows a menstrual pattern, while uterine fibroids may cause pressure or heavy periods. An ovarian cyst can cause pain on one side, particularly if it becomes irritated or ruptures. Pregnancy-related pain, including ectopic pregnancy, needs urgent assessment when it occurs with bleeding, dizziness, or one-sided pain.

Pain is not a diagnosis by itself. A clinician may ask about your periods, contraception, pregnancy possibility, discharge, urinary symptoms, bowel symptoms, and whether the pain is new or recurring. Examination and tests are chosen according to that pattern.

Stop or change the activity when pain starts, and avoid positions or depth that trigger it. Gentle communication with your partner, more time for arousal, and a suitable lubricant may reduce friction-related discomfort. Note when the pain happens, where you feel it, how long it lasts, and any bleeding, discharge, fever, urinary change, or bowel symptom. Arrange a medical appointment if the pain keeps returning, affects intimacy, or does not settle after simple changes. Do not self-treat possible infection with leftover medication.

Seek urgent medical care for sudden or severe lower abdominal pain, fainting, marked dizziness, shoulder-tip pain, heavy vaginal bleeding, fever with worsening pain, repeated vomiting, or a rigid or very tender abdomen. If pregnancy is possible, pain with bleeding, weakness, or one-sided discomfort needs prompt assessment because an ectopic pregnancy can become dangerous. New pain after a positive pregnancy test should not wait for a routine appointment.
