Lower abdominal pain during fertile days

Some people notice a one-sided ache or brief cramp around the middle of the cycle as an ovary releases an egg. The timing can help describe the symptom, but it does not prove that ovulation is the cause. Endometriosis, adenomyosis, fibroids, ovarian cysts, and pelvic inflammatory disease can also cause pelvic pain that happens to overlap with fertile days. Pain with discharge, fever, bleeding, pain during sex, or bowel and bladder changes needs attention beyond cycle tracking.

Keep a cycle diary showing the first day of bleeding, estimated fertile days, pain side and severity, duration, and associated symptoms. Rest, warmth, hydration, and an over-the-counter pain medicine that is safe for you may help mild discomfort. Arrange a clinical review if the pain returns across cycles, lasts longer than expected, interferes with activities, or concerns you about fertility or pregnancy.

Seek urgent care for sudden intense or worsening pain, faintness, dizziness, shoulder-tip pain, fever, persistent vomiting, or heavy bleeding. If pregnancy is possible, one-sided pain with bleeding should be assessed promptly because ectopic pregnancy can be serious.
