What should you know about loud barking cough?

A barking cough may occur when the voice box and nearby airway become inflamed. A viral illness such as croup is a common pattern in children, often with hoarseness and symptoms that worsen at night. A cold, acute bronchitis, bronchiolitis, asthma, allergic rhinitis, or acid reflux can also produce cough, although the sound alone cannot identify the cause.

Notice whether the cough began suddenly, whether there is fever, a runny nose, wheezing, a harsh noise when breathing in, or difficulty speaking, drinking, or settling. A clinician may need to assess the person rather than relying on the cough description.

Keep the person calm and sitting upright, because crying and agitation can make noisy breathing harder. Offer small sips of fluid if swallowing is comfortable. Follow advice from a clinician about fever or other symptoms, and avoid giving cough medicines or leftover antibiotics without guidance. Do not use steam that could burn a child.

If the cough is mild and breathing is comfortable, monitor the breathing pattern, voice, fluid intake, and alertness. Arrange a medical assessment if the cough is persistent, repeatedly disrupts sleep, or follows choking or a possible inhaled object.

Call emergency services immediately for struggling or rapidly worsening breathing, blue or grey lips, pauses in breathing, severe drowsiness, inability to speak or cry normally, drooling with difficulty swallowing, or a harsh breathing noise while resting. Sudden onset after food, medicine, or a sting may indicate a serious allergic reaction. Do not force food or drink when breathing or swallowing is unsafe.

Ask what is causing the cough, whether the airway sounds narrowed, which symptoms should trigger urgent review, and how to manage fever or discomfort. Tell the clinician when it started and describe breathing while resting and sleeping.
