Short answer

Loose skin under the buttocks can reflect skin laxity, changes in underlying volume, or a gluteal fold concern that needs assessment.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Anatomy
The fold reflects skin, fat, muscle, and connective tissue
Self-care
Limit rubbing and manage moisture in the crease
Assessment
Separate gradual laxity from pain, swelling, or a lump

What may be changing

The crease beneath the buttocks is shaped by skin, fat, muscle, connective tissue, and the way the upper thigh meets the buttock. Weight loss can leave a softer fold, while ageing and sun exposure can reduce skin support. Repeated rubbing, sweating, tight clothing, or prolonged sitting may cause tenderness and irritation. A new lump, marked asymmetry, or painful swelling is a different issue from gradual laxity and should be examined.

Ways to approach it

Maintain a stable routine of movement and strength work if appropriate for your health, but remember that exercise changes muscle and contour rather than removing loose skin. Reduce friction with breathable clothing and keep the crease dry if it becomes irritated. For selected mild laxity, a clinician may discuss Radiofrequency Skin Tightening or HIFU Skin Lifting. A Thread Lift has specific indications and is not a general solution for every buttock fold. Marked excess skin or a contour problem may call for a surgical opinion. Ask for an assessment of the fold, nearby nerves, scars, and sitting or activity needs.

When to seek care

Seek medical review if swelling appears quickly, pain is substantial, the skin becomes hot or increasingly red, or a lump persists. Urgent care is needed for fever with spreading redness, severe pain, drainage, or skin breakdown. Recurrent moisture, cracking, or itching should be diagnosed so a rash is not mistaken for cosmetic laxity.

Questions for your doctor

Is the fold caused mostly by skin, fat, muscle shape, or irritation? Which treatment fits the exact area beneath the buttock? Could treatment affect sitting, exercise, or sensation? How many visits might be needed, and what recovery is expected? What would make surgery a more appropriate option?

Questions people ask

Can squats remove loose skin under the buttocks?

Strength training may improve muscle support and contour, but it does not remove excess skin. The cause of the fold should be assessed if it bothers you.

Is a non-surgical treatment suitable for this area?

It may be suitable for some people with selected laxity, but the exact fold, skin quality, and medical history determine the plan.
