What should you know about long lasting orthostatic hypotension?

Short answer
Long lasting orthostatic hypotension causes blood pressure to fall after standing, leading to dizziness or faintness that needs assessment when it persists or affects safety.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Main symptom
- Dizziness or faintness after standing
- Safety step
- Sit or lie down as soon as symptoms begin
What happens on standing
When you stand, blood normally shifts toward the legs and the body adjusts circulation. With orthostatic hypotension, that adjustment may be insufficient, so the brain briefly receives less blood flow. Dizziness, blurred vision, weakness, nausea, or fainting can follow. Dehydration, illness, prolonged bed rest, heat, and some medicines may contribute.
Repeated episodes can increase fall risk and may point to a problem that needs treatment. Record what you were doing, how quickly symptoms began, whether you fainted, and any recent medicine or illness changes. A clinician may compare blood pressure and pulse while you are lying down and standing.
Reducing symptoms safely
Rise in stages: sit first, move your ankles, then stand while holding a stable surface. Drink regularly unless a clinician has restricted fluids, avoid prolonged standing in heat, and sit or lie down as soon as symptoms start. Keep walkways clear and use support if balance is unreliable.
Do not increase salt, alter medicines, or use compression garments without individual advice. A GP Consultation can review possible causes, medicines, hydration, and fall risk.
When to seek care
Seek urgent help after fainting with injury, chest pain, severe breathlessness, new weakness, confusion, or a fast or irregular heartbeat. Arrange prompt review if episodes are becoming more frequent, occur without standing, or prevent safe walking, bathing, or driving. Repeated vomiting, diarrhoea, or poor fluid intake also merits timely advice.
Questions for your doctor
Ask which medicines or conditions could be contributing, how to measure blood pressure at home, and whether you need blood tests or heart assessment. Ask what activity and fluid plan is suitable for you.

Questions people ask
Why do I feel dizzy when I stand?
A blood pressure drop after standing can briefly reduce blood flow to the brain and cause dizziness.
Can medicines contribute to orthostatic hypotension?
Some medicines can affect blood pressure or fluid balance. A clinician should review them before any changes are made.
