Liver pain during ovulation: what to know

Ovulation pain usually occurs low in the pelvis and may affect one side. Discomfort under the right ribs or in the upper abdomen can instead involve the abdominal wall, bowel, gallbladder, liver or another nearby structure. Hormonal changes may make existing pelvic conditions more noticeable, but pain in this area should not be automatically attributed to ovulation. Endometriosis can cause cyclical pain, while an ovarian cyst can cause pressure or sharper pelvic discomfort; neither diagnosis can be made from location alone.

Write down when the pain begins, where it spreads, how long it lasts, and whether meals, movement, urination or bowel movements affect it. Note your cycle day and any nausea, fever, bloating, unusual bleeding or yellowing of the skin. Rest, drink fluids and use a warm pack if that is comfortable. Arrange a clinical review for recurring upper-abdominal pain, especially if it is becoming stronger or no longer follows a cycle pattern.

Seek urgent medical help for sudden severe pain, fainting, confusion, repeated vomiting, a rigid or swollen abdomen, trouble breathing, high fever, or yellow skin or eyes. Urgent assessment is also appropriate for pain with heavy vaginal bleeding, a missed period, or possible pregnancy because several abdominal conditions need prompt evaluation.
