Possible explanations

Cramps happen when the uterus contracts, but unusually strong pain may be linked to adenomyosis, fibroids, polyps, or changes in the uterine lining. Perimenopause can make bleeding patterns less predictable. Polycystic ovary syndrome can also disrupt ovulation and cycle timing. The amount of blood does not measure how serious pain is, and a light period does not rule out a condition affecting the uterus or pelvic organs.

Pain that begins suddenly, steadily worsens, occurs between periods, or affects sex, bowel movements, work, or sleep merits a clinical assessment. Anaemia is more often linked with ongoing heavy bleeding, but tiredness, breathlessness, or paleness should still be discussed.

Keep a record of pain location, severity, timing, bleeding, bowel or bladder symptoms, and medicines taken. Heat, rest, gentle movement, and an appropriate pain medicine may help some people; check the label and ask a pharmacist if you have stomach, kidney, bleeding, or pregnancy concerns. Arrange a review if pain is new, recurrent, or not controlled with usual measures.
