What should you know about lifted toe nails?

When a toenail separates from the skin underneath, the gap can appear pale, yellow, or grey. Repeated pressure from narrow shoes, running, a stubbed toe, picking, or a previous injury may start the process. Nail fungus and nail psoriasis can also affect the nail plate and its attachment. One lifted toenail after an obvious knock differs from gradual lifting of several nails, so the pattern and surrounding skin help guide assessment.

The lifted portion does not reattach to the existing nail bed. It must grow forward and be replaced by new nail from the base. Toenails grow slowly, so the visible gap can remain while the nail advances. Keeping the toe protected reduces the chance of catching or tearing the nail.

Trim the loose part straight across only when it can be done without pain, and use clean clippers. Choose roomy shoes and moisture-wicking socks, dry carefully after washing, and avoid digging under the nail. Do not tear the nail away or apply glue, polish, or a false nail over a separated area. A clinician can check for trauma, skin inflammation, and infection, and may test nail material before selecting treatment.

If the toe was injured recently, protect it from pressure and watch for worsening tenderness or swelling. Avoid sharing nail tools, and clean tools between uses. Treatment for a confirmed cause may be topical or oral medicine, or another approach chosen after examination.

Seek prompt care for severe pain, marked swelling, pus, bleeding that does not settle, fever, or redness spreading around the toe.
