What should you know about an LH surge and ovulation pain?

Luteinising hormone rises before an ovary releases an egg. A home urine test may detect this rise, but it cannot prove that ovulation definitely occurred or explain every episode of pain. Ovulation pain can feel like a twinge, ache or sharper sensation on one side and may change sides in different cycles. Pain timing can be useful for fertility tracking, yet it is not a reliable way to diagnose a gynaecological condition.

Record the first day of each period, the test result, which side hurts, how long the pain lasts and any bleeding or discharge. Gentle heat and rest may help. Use pain relief only if it is suitable for you and follow the label or a clinician's advice, especially if pregnancy is possible. If you are trying to conceive, discuss how to combine LH testing with other fertility signs rather than treating one result as definitive.

Arrange medical assessment for pain that is severe, persistent, recurrent outside the mid-cycle window, or associated with heavy bleeding, unusual discharge, fever, vomiting, faintness or pain during sex. Seek urgent help for sudden intense pelvic pain, collapse or possible pregnancy with pain or bleeding. Conditions such as ovarian cysts or endometriosis can also cause pelvic pain, so the timing of an LH surge does not rule them out.
