What the test looks for

Large granular lymphocytes are immune cells that can be seen in blood or bone marrow. Flow cytometry passes prepared cells through a laser-based analyser and records proteins on their surface or inside them. The pattern helps a laboratory identify cell populations and whether they appear unusually uniform.

An LGL population can occur with infection, inflammation, autoimmune disease or a blood disorder. A flow result is therefore one part of the assessment, not a diagnosis in isolation. The specialist may compare it with the blood count, blood-film findings, symptoms and other laboratory tests.

What happens next

The sample may be blood, or it may come from a bone marrow biopsy if the team needs to examine blood-forming tissue more closely. Follow the laboratory instructions and tell the team about medicines, infections, fevers, repeated infections, bruising or unusual tiredness.

Ask when the result is expected and whether additional testing is needed to assess persistence or clonality. Management depends on the complete picture; some reactive findings are observed while a confirmed disorder may need specialist monitoring or treatment.

Contact your healthcare team promptly for fever, chills, repeated infections, worsening breathlessness, unusual bleeding, or marked weakness while this evaluation is underway. Seek emergency help for severe breathing difficulty, fainting, confusion, or bleeding that does not stop.

Ask whether the sample was blood or marrow, what cell markers were found, whether the pattern is reactive or clonal, and which other results are still pending. Ask what symptoms or blood-count changes should lead to an earlier review.

Does an LGL flow-cytometry result confirm cancer?

No. The result must be interpreted with symptoms, blood counts and other tests; reactive immune changes can produce an LGL population.

Is flow cytometry the same as a bone marrow biopsy?

No. Flow cytometry is a laboratory analysis of cells, while a bone marrow biopsy is a procedure that collects marrow for several forms of examination.
