What the levels mean

Clinicians may describe heart failure by how much ordinary activity brings on breathlessness, tiredness or swelling. An earlier level may cause symptoms only with more strenuous exertion; a more advanced level can cause symptoms during everyday tasks or at rest. Another classification looks at the heart’s pumping measurement and whether it is reduced, mildly reduced or preserved. These descriptions answer different questions, so one label does not tell the whole story.

Symptoms such as a cough, fatigue, palpitations or swollen ankles can occur with heart failure, but they can also have other causes. A clinician combines your history, examination, blood tests and heart tests to understand the pattern. High blood pressure and atrial fibrillation can contribute to heart strain and may need attention alongside heart failure.

Take prescribed medicines on the schedule given and keep a record of changes in breathing, energy, sleep and swelling. Follow the fluid, salt and activity advice tailored to you; do not change medicines or fluid intake on your own. When to seek care

Seek emergency help for severe or rapidly worsening breathlessness, chest pressure, fainting, confusion, blue lips or an inability to speak in full sentences. Contact your care team promptly if swelling increases, breathing becomes harder when lying down, you wake gasping, or your usual activity suddenly becomes difficult. These changes need assessment rather than being judged by a previous level.
