Painful leg swelling during pregnancy

Pregnancy can cause ordinary swelling in both legs, but pain changes the picture. A sore calf with swelling, warmth, redness, or tenderness on one side can signal a circulation problem that requires urgent assessment. Cramp, muscle strain, pressure from footwear, and varicose veins are other possibilities. You cannot distinguish these causes reliably by appearance or by pressing the area. Pregnancy also changes clot risk, so do not dismiss a new painful swelling as routine fluid retention.

Contact your maternity unit or an urgent medical service for advice about new painful swelling. Keep activity gentle and follow the instructions you receive. Do not massage the leg, apply vigorous heat, or take someone else’s anticoagulant or pain medicine. If you are waiting for advice, note when symptoms began, which leg is affected, and whether redness or warmth is spreading. Keep your pregnancy details and current medicines available.

Call emergency services for sudden breathlessness, chest pain, coughing blood, collapse, or severe dizziness. Seek same-day assessment for one-sided swelling with calf pain, warmth, redness, or tenderness, even if breathing is normal. A severe headache, visual symptoms, or sudden swelling of the face and hands also warrants urgent maternity contact because pregnancy complications can present with those changes.

Ask whether you need an examination or imaging, what movement is safe while waiting, and which pain relief is suitable in pregnancy. Mention any previous clot, recent travel or immobility, varicose veins, and medicines that affect clotting.
