Leg swelling while pregnant

Short answer
Leg swelling while pregnant is often caused by normal fluid shifts and pressure on the veins, although sudden swelling or other symptoms should be checked by a maternity professional.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Often seen
- Gradual puffiness in both ankles, worse after standing or later in the day
- Prompt review
- Fast change, one-sided symptoms, or swelling with headache or visual disturbance
Why it happens
Pregnancy increases the amount of fluid in the body, and the growing uterus can make it harder for blood to return from the legs. This may cause gradual puffiness in both feet and ankles, often more noticeable after a busy day or in warm weather. Visible veins can become more uncomfortable as well. Swelling by itself does not diagnose a complication. The timing, symmetry, speed of change, and accompanying symptoms are more useful than appearance alone.
Comfort measures
Avoid remaining in one position for long periods, take gentle walks if approved, and flex your ankles while sitting. Rest with your legs supported and wear comfortable shoes. Side-lying rest may feel better than lying flat. Keep routine antenatal appointments and mention swelling there, even when it is mild. Do not take medicines for fluid loss or use compression garments without checking with your maternity clinician.
When to contact maternity care
Call your maternity team promptly for rapid swelling, swelling mainly on one side, or leg pain, warmth, redness, or tenderness. Seek emergency care for breathlessness, chest pain, faintness, or coughing blood. A severe headache, visual disturbance, pain high in the abdomen, or sudden swelling of the face and hands also needs urgent pregnancy assessment. Do not wait for a routine visit when these symptoms appear.
Discuss at your appointment
Ask whether your blood pressure and urine should be checked, whether your activity is appropriate, and whether support stockings would help. Tell your clinician about previous circulation problems, a clot, new medication, or swelling that is getting worse.
